Depending on your requests - referencing, disclosing, revising, correcting, adding, deleting and terminating the usage of your personal information, we may need to confirm your identification. In such case, we will send you the documents such as “Disclosure Request Form” or “Correcting Information Form” . Please pay postage for sending us your documents.
When you fill in and return such documents to us, please include two of the following copies for identification ; your driver’s license, your National Health Insurance card, your passport, or your residential certificate.

Disclosure Fee

In accordance with the article 30 of the Personal Information Protection Laws, you are required to pay us 500 yen per information. Please include a 500-yen postage stamp at the time of your disclosure request.

Changes on Our PI Policy

Our basic policy on personal information may change from time to time due to changes of laws or ordinances.
